First-time Patients

For first-time patients to this hospital and patients with a letter of referral from another hospital, please come to the reception desk.

First Visit Procedures

Please fill out the Patient Registration Form and submit it to the reception desk on the 1st floor of the outpatient wing along with your Health Insurance Card and Letter of Referral.

After you checking in at the reception desk, please follow the guidance and wait in front of the consultation room.

When it is your turn, we will ask you to come to consultation room.

After the consultation, please pay your bill at the Accounting Counter on the first floor of the outpatient wing.

Please go home safely.

Returning Patients

Registered patients can use the returning patient reception machine. Please check in at the return patient reception machine.

Emergency Patients

Emergency Visit Procedures

Please call the emergency contact (TEL: 049-283-0019) before visiting. Please explain your request for an emergency outpatient consultation and provide the following information.

Name, Date of birth
Patient ID Card number (if you have one)
Symptoms, Conditions

Please bring your Health Insurance Card, Patient ID Card (if you have one) and call the reception desk.

In principle, medical care is provided to emergency outpatients in the order of arrival.　However, the order may change depending on patient symptoms and degree of urgency. Thank you for your understanding.

Accounting is carried out at the reception desk.
When visiting outside regular reception hours, a deposit is required. Only cash is accepted. Payment for medical care outside regular reception hours is as follows.

Deposits outside regular reception hours

The following amount must be paid after consultation.
- For patients who can present their health insurance card: 10,000 yen.
- For patients involved in a traffic accident, work-related injury (industrial accident) and patients who cannot present their health insurance card (including persons who are uninsured): 30,000 yen
Please come to the office to settle any over- or under-payment on the following Monday through Saturday between 9:00 a.m. and 5:00 p.m. (excluding days when the hospital is closed).
Only cash is accepted.

Medicine

Medications are minimally prescribed. In principle, prescriptions provided at night or on a non-consultation day are handled as emergency care, and only a two-day supply of medication can be provided. Please visit the outpatient department later.
